During Covid we will now be registering patients however with restricted times to come to the surgery.
The registration times will be: 8 - 9 am, 12 - 2pm & 4 – 5 pm.
This is to make sure we adhere to strict social distancing and monitoring the numbers of patients in the waiting room to protect patients and staff.
Please download and complete the attached forms. Bring these to the surgery in the times above along with your Photo ID (if possibly driving licence) and proof of address.

Patients must attend the practice if they wish to register with us. We only accept patients who live within our practice area boundary. Please click here to see the map.
We endeavour to ensure fair treatment for all our patients and we do not discriminate on grounds of race, sex, religion, disability or illness. Patients will be registered with one of the doctors in the practice. However, under the terms of the current GP contract patients may specify their preference if they wish to register with a particular doctor. If that is not possible an explanation will be given.
All new patients will be asked if they wish to have a Summary Care Record. This is an electronic record which will give the out-of-hours and Accident and Emergency doctors access to the medications you take, any allergies you have and any adverse reactions to medications. This will allow health care staff to provide you with safer care in an emergency, when the practice is closed or when you are away from home in another part of England.
If you wish to opt out of this, please advise reception and they will give you an opt-out form to complete. Children will automatically have a Summary Care Record made for then. If you do not want your child to have a Summary Care Record you will need to fill in an opt-out form on behalf of your child and return it to us.
