Times for accepting new registrations:
The registration times will be: 8.00am to 9.00am, 12.00pm to 2.00pm & 4.00pm to 5.00pm
Forms can be obtained by either dropping into the surgery and collecting a pack or download and complete the attached forms.
Bring these to the surgery in the times above along with your Photo ID and proof of address.

Patients must attend the practice if they wish to register with us, bringing proof of identification and address. We only accept patients who live within our practice area boundary. See the list on our Practice Area page.
We endeavour to ensure fair treatment for all our patients and we do not discriminate on grounds of race, sex, religion, disability or illness. Patients will be registered with one of the doctors in the practice. However, under the terms of the current GP contract, patients may specify their preference if they wish to register with a particular doctor. If that is not possible an explanation will be given.
All new patients will be asked if they wish to have a Summary Care Record. This is an electronic record which will give the out-of-hours and Accident and Emergency doctors access to the medications you take, any allergies you have and any adverse reactions to medications. This will allow health care staff to provide you with safer care in an emergency, when the practice is closed or when you are away from home in another part of England.
If you wish to opt out of this, please advise reception and they will give you an opt-out form to complete. Children will automatically have a Summary Care Record made for then. If you do not want your child to have a Summary Care Record you will need to fill in an opt-out form on behalf of your child and return it to us.
Temporary registrations
Please note that whilst we are happy to register you as a temporary patient, we will not be able to offer you an appointment for 48 hours with our clinicians. We are not able to make referrals to other services for temporary patients this will need to be made through your own GP Practice when you return to your permanent address. If your problem is urgent please use NHS 111 or 999.
Temporary Registrations will be for a maximum of three months. We reserve the right to remove you from our register prior to this time. We reserve the right to refuse registration.
